Community Care was established to assist patients who do not qualify for Medical Assistance (Medicaid) and whose annual incomes are at or below 275% of the Federal Poverty Level. The financial application asks for information on family size, employment, income, banking and insurance. If patients meet the guidelines, their total balance will be zero. An eligible individual will be covered by the program for up to one year barring any significant change in income. Patients must apply for Medicaid and be found ineligible before they qualify for any Community Care Assistance. (Review income level guidelines)
