You may register in person the day of your visit. Please arrive at least 15 minutes prior to your scheduled appointment.

Assignment of benefits
Acknowledgement of your receipt of our
Notice of Privacy

You will be asked for a picture ID and current insurance information each time you check in for an appointment. Please alert us to any changes in insurance or personal information.

We appreciate your assistance in helping us maintain accurate and up-to-date information for your medical chart and billing records. This helps assure the timely processing of your insurance claims and medical records.
